You dont, you need 2 of those. it can happen through rebellions, but it can also happen when a kingdom is formed and loses land making it smaller. You need a Duchy and a lot of counties that do not form a duchy.

Essentially, a grand duchy is a kingdom-tier title without some of its benefits and restrictions (namely those derived from official coronations).
