What are your thoughts overall on these types of programs? There only seem to be a few of these programs, all at extremely competitive hospitals. ...I did a month-long derm rotation(am a EM resident) during Covid and saw a lot of telemedicine and the idea of doing a telemedicine only practice in Derm seems like taking all the worst parts of Derm and doing that all day long.

I think you should definitely still pursue! This is potentially a lifelong decision, and some programs realize you are more than one relatively unrepresentative score. I know a dermatology resident at my home institution who matched directly out of med school with a Step 1 score of 223.Matched well in the end with no research year. If you actually want to do research and can convey that, you will match to a 2+2 without a derm research year; it is a very small applicant pool that wants to do that, truly. The point of the derm research year in all honesty is to make connections, not to learn how to conduct research.

That being said, a research year should afford you some free time to work on the side in gigs like tutoring for high school or MCAT/college. I think you could easily earn $40/hr tutoring high school and do that 3x a week for 2 hrs each time. That adds up to about $10k after everything, which is basically how much stipend most programs will pay ... Like many of my co-applicants my goal is simply to match into Derm. I am completely not picky about the program or the region. Sure there are some programs and regions that I slightly prefer, but overall my goal is to match "somewhere."
